Bilateral Iridoschisis and Unilateral Angle Closure Glaucoma

4SB Ankara Ulucanlar Göz Eğitim ve Araş. Hast. 2. Göz Kliniği, Klinik Şefi, Uzm. Dr. Iridoschisis is a rare degenerative condition of the iris stroma that occurs bilaterally in elderly patients. It is usually associated with angle-closure glaucoma.
A-45 year-old woman presented to the ophthalmology department with change in the color of the left eye. Her bestcorrected visual acuity was 45/50 in both eyes. Intraocular pressures with applanation were 16 mmHg OD and 25 mmHg OS, respectively. On slit-lamp examination, more in the left eye, atrophy of the iris and fibrillary degeneration of the iris stroma were seen bilaterally. Fundus examinations of both eyes were normal. On gonioscopy, grade 2 angle in both eyes was determined with peripheral anterior synechiae. Nd-YAG laser iridotomy and topical antiglaucomatous therapy were performed on the left eye.
Angle-closure glaucoma with forming peripheral anterior synechiae occuss in about 90% of iridoschisis cases, but this condition could be asymmetric the affected eye must be treated and the other eye must be followed in this condition. Keywords : Iridoschisis, glaucoma
