Results of Trabeculectomy with Mitomycin C

Dr. Purpose: To evaluate the effect of using mitomycin C (MMC) on the results of trabeculectomy in glaucoma patients with a high risk of surgical failure.
Materials and Methods: Twenty-two patients admitted to our clinic between 2004 and 2008 with intraocular pressures (IOP) higher than 21 mmHg despite maximally tolerated medical treatment were enrolled in this study. Twenty-seven eyes of the 22 patients (8 males, 14 females) underwent trabeculectomy with application of subconjunctival MMC of 0.2 mg/ml concentration for 2 minutes.
Results: Mean age of the patients was 47.27±22.25 years. All patients were followed up for a mean period of 19.33±13.77 months. The mean preoperative IOP of the patients was 30.22±8.58 mmHg. Primary angle-closure glaucoma was present in 8 eyes, congenital glaucoma in 6, primary open angle glaucoma in 5, neovascular glaucoma in 2, pseudoexfoliative glaucoma in 2, glaucoma secondary to keratoplasty in 1, pseudophakic glaucoma in 1, aphakic glaucoma in 1, and glaucoma secondary to pars plana vitrectomy in 1. At their last visit, IOP was less than 21 mmHg in 26 eyes (96.3%), and in 8 of them (35.2%) postoperative anti-glaucomatous drugs were being used. There were no complications such as shallow anterior chamber, choroidal detachment, hypotony-associated maculopathy, or phthisis bulbi to deal with.
Conclusion: In high risk patients, applying suitable doses of MMC for a suitable period during trabeculectomy increases surgical success rates without affecting complication rates. Keywords : Glaucoma, mitomycin C, trabeculectomy
