New Developments in Seton Surgery

Modifications in size and shape and introduction of valve systems have taken place since 1970 when Molteno introduced the prototype of the tube-plate type implant. The subconjunctival reservoir induced by the explant usually loses its efficacy within 5 years due to the proinflammatory agents in the aqueous humor in this type of implantation. For this reason, new implants that do not contain long tubes and explants that do not require bleb formation are being evolved. The drainage capacity of the Schlemm canal, intrascleral area, and suprachoroidal space are investigated as these are potential target tissues for these developing implants. Recent studies have shown that tube implantation is as effective as filtration surgery.
