Anatomy, Pathophysiology and Gonioscopy in Narrow Angle Glaucoma

The most important stepin making the diagnosis of narrowangle glaucoma is to perform good gonioscopy. Angle elements consist of peripheral iris, ciliary body, non-pigmented and pigmented trabeculum, scleral spur and Schwalbe's line. Angle width and closure risks are evaluated by Schaffer, Scheie or Spaeth grading systems. The difference between temporary and permanent angle closure is made by indentation gonioscopy. The treatment approach differs accordingly. It is possible to evaluate angle with the modern advanced instruments but gonioscopy is the gold standard for angle evaluation.
